| It has been 5 years since the third edition of the Handbook of Dialysis was first printed, and since that time, translations have appeared in many languages, including Chinese, Japanese, and Russian. We are particularly gratified by the strong international interest in this text. In putting together this fourth edition, we were most respectful of the concept that nephrology is now very much international. KDOQI guidelines are being incorporated into KDIGO, and practice patterns are becoming more uniform. Differences between US and international nephrology remain, but the international experience is enriching our knowledge and practice base. Given this strong international interest, in this fourth edition, all measurement values and examples were given both in English units and SI values. A chapter is included on hemodiafiltration, which remains a niche therapy for chronic treatment in the United States, but which is much more widely used in the rest of the developed world.Since the third edition, progress in dialysis has been incremental. Much more attention is being paid to predialysis, stage 1-4, CKD (chronic kidney disease). Accordingly, this area is covered in more depth. In previous versions, issues pertaining to water treatment for dialysis had only been summarized. In this edition, particularly with suggestions that patient results improve with use of ultrapure dialysate, more extended discussion is provided. Much progress has also been made in the field of obtaining vascular access, especially in methods of assuring arteriovenous fistulas for patients who previously might have received either venous catheters or grafts. These sections were entirely rewritten, and access was divided into two separate chapters, one focusing on catheters and one on fistulas and grafts.The use of continuous therapies has undergone much simplification due to availability of new machines. Some of the earlier, do-it-yourself approaches to CRRT were accordingly deleted. We added a chapter on how to provide short daily and long nocturnal dialysis, a requirement as more frequent dialysis schedules become more popular both in the United States and abroad. Advances in the area of peritoneal dialysis have been gradual since the appearance of the third edition. Providers have become perhaps less interested in quantification of dose and more concerned about glycemic load and ultrafiltration, and now new PD solutions using alternative osmotic agents are widely available. Our European colleagues were particularly helpful in reviewing this area. Knowledge of bone disease and methods to control both hyperphosphatemia and hyperparathyroidism has grown enormously since the third edition, and this new information, including the latest guidelines, is emphasized. In each chapter, we have tried to incorporate the latest information available and to include information in the 2006 KDOQI guideline updates. Web links at the end of many chapters will help ensure that the book remains current as new findings and treatments emerge in the years to come.Although a flood of information has become available pertaining to all aspects of medicine, we have tried to maintain the unique character of the Handbook of Dialysis, explaining each area from the basics, but then progressing to an advanced level of understanding, and keeping the discussion at a practical level, realizing that texts like the Handbook of Dialysis are often relied upon by both new and experienced nephrology care providers to help them in their difficult job of providing the best care for our patients.John T.
